Membership Policies
Simple terms, clear expectations, and thoughtful care.
Membership services are provided by appointment and are subject to provider availability. Services must be used within the active membership period and do not roll over to the next billing cycle.
Term + Payment
Membership is $245 per month and is billed every 30 days to the payment method provided at enrollment. The plan requires a 3-month minimum commitment and auto-renews monthly.
Cancellation
Members may cancel after completing the initial 3-month minimum term by providing 30 days’ written notice. Cancellation requests may be submitted by email or delivered in person to the clinic.
Early Termination
Early termination prior to completing the 3-month commitment is permitted; however, the remaining balance for the initial term must be paid in full at the time of cancellation.
Missed Appointments
Appointments missed without at least 24 hours’ notice may be considered redeemed and cannot be rescheduled or rolled over.
Non-Transferability
Memberships are exclusive to the individual who enrolls and may not be transferred, shared, or sold to another party.
Discount Limitations
Discounts apply only to full-priced products and services. Promotions, packages, and sale items are excluded from additional membership discounts.
